Bring your clinical expertise and passion for providing quality, compassionate care to a challenging and rewarding role as a Registered Nurse In Practice, The Chilliwack and Fraser Health Rural Primary Care Centre: Momíyelhtelaxwt (“Helping one another.”)
Across British Columbia, Divisions of Family Practice, Fraser Health, First Nations, and community partners are working to establish primary care networks (PCNs). PCN Health Centers are community-based network of Family Physicians, Nurse Practitioners, Registered Nurses, Clinical Pharmacists, Traditional Wellness Mentors, and allied health professionals working together to provide coordinated health in one geographical location.

The centre will be launched in Spring 2022 to residents in Chilliwack, Agassiz, Hope and the Fraser Canyon. This area includes twenty-two Indigenous communities from Chilliwack to Boothroyd. The PCN Momíyelhtelaxwt Health Centre attributes include promoting access and attachment to quality health care to all populations with a specific focus on those who are particularly challenged in accessing primary care. This includes aging and rural populations, struggling with homelessness, substance use, mental health, patients with multiple chronic diseases, complex health needs and needing support with other longitudinal health care needs. The centre will provide culturally safe and respectful care by incorporating Traditional Healers, Traditional Wellness Mentors as well as integrating Indigenous resources that reflect on traditional teachings by the Indigenous communities.

Be part of a cohesive team where each provider has their own unique role and scope in the provision of patient care and you work together to make sure that the patient’s needs are met in the most effective way possible.

Responsibilities would include:

• Delivering direct patient care to clients of family practice clinics within the Primary Care Network
• Performing comprehensive and focused health assessments, outreach, referrals and health counselling including education, self-management and wellness promotion.
• Ensuring clients, families and caregivers have access to timely, person-centred, continuous and culturally safe quality health services
• Collaborating with the interprofessional team in developing a comprehensive shared client care plan
• Supporting clients and their families by providing information through their decision-making process regarding treatment options
• Participating in research and quality improvement programs
Are you motivated to join us? We will be looking for you to have:
• Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
• One (1) year recent related clinical experience including assessing and treating children, youth and adults in an acute or community/outpatient care setting
• Immunization additional education completed and up to date
• Valid BC drivers license and access to a personal vehicle for business related purpose
