Role and Facility Information

The Nurse Practitioner Primary Care Clinic (Axis NP-PCC) is looking for a 0.5 FTE Nurse Practitioner to join our team as we build healthier communities and develop a network of outstanding healthcare professionals. The clinic is set to open in September 2020.

The Axis NP-PCC is operated in partnership with the Ministry of Health, the Surrey-North Delta Division of Family Practice, Fraser Health Authority and the Association of the Nurses and Nurse Practitioners of BC. NP-PCC attributes include:

• Implementing processes to ensure all people in a community have access to quality primary care and are attached to a primary care provider within their community
• Delivery of extended hours of care, including early mornings, evenings and weekends
• Delivery of same-day access for urgently needed primary care through the NP-PCC
• Delivery of comprehensive primary care services through a team-based care approach
• Collaborative and co-ordinated care with diagnostic services, hospital care, specialty care and community services for all patients


NPs will be required to sign a PCN contract with the health authority and a practice agreement with the NP-PCC.


Be part of a cohesive team where each provider has their own unique role and scope in the provision of patient care, and you work together to make sure that the patients’ needs are met in the most effective way possible. Responsibilities will include:

• Performing comprehensive assessment of patients including diagnosing diseases, disorders and conditions
• Initiating treatment including health care management, therapeutic interventions and prescribing medications
• Providing medical coordination and participation in multidisciplinary team planning for the ongoing health needs of patients
• Participating in the development and operation of a team-based practice wide program that may include:
o Health promotion/disease and illness prevention
o Chronic disease management
o Specialty care of specific populations (e.g. frail elders, terminally ill, pregnant patients, children)
• Clinical teaching/preceptorship
• Clinical administrative services, including:
o Health care/service planning activities including participating in planning of long-term health care delivery goals for practice
o Participation in the evaluation of the efficiency, quality and delivery of the service (e.g. participation in medical audits, peer and interdisciplinary reviews, chart reviews, and incident report reviews, quality improvement activities)

Additional advanced skills and competencies will be considered with the goal of building a team of NPs with diversity in expertise. Some of these may include:

• Advanced prescribing (e.g. opioid agonist therapy, antiretroviral therapy)
• Advanced procedures (e.g. IUD insertion, advanced wound debridement)
• Advanced practice with specialty populations (e.g. seniors, pediatrics, Indigenous populations)

Are you motivated to join us and be one of the first NPs in BC to be part of this new model of care? We will be looking for you to have:

• Current or anticipated provisional/practicing registration as a Nurse Practitioner with the BC College of Nursing Professionals (BCCNP)
• Relevant, clinical nursing experience
• Ability to work in a trauma-informed capacity
• Completion of a BCCNP approved controlled drug and substance prescribing course
