Role and Facility Information

Vancouver Community Health Services is seeking a Geriatrician for a shared 1.0 FTE position with one other geriatrician to work permanent part time (0.5 FTE) starting January 4, 2021.

The Short Term Assessment & Treatment (STAT) Centre is a specialized unit for seniors experiencing health changes that interfere with functioning and independence. Services are provided by an interdisciplinary team with a holistic approach to client-centred care. The STAT Centre Program is composed of a Day Hospital and a 17 patient In-Patient Unit.

The STAT Centre Day program is located at Vancouver General Hospital. The In-Patient program has temporarily moved to GF Strong to accommodate VGH COVID Planning. The priority at this time is to cover the In-patient unit, but based on availability, some coverage at Day Hospital Program would be helpful.

The geriatricians working in this program collaborate with multidisciplinary teams of health care professionals to serve a vulnerable population. The Day Hospital provides a patient-centred, geriatric-friendly environment where assessments and management can be done thoroughly and progress can be monitored. STAT Unit inpatients are increasingly complex with medical and psychiatric co-morbidities as well as functional challenges. These patients require a high level of support as they undergo medical and psychiatric assessments and management. These patients are under the primary care of a GP MRP and require the oversight of a consulting geriatric medicine specialist to monitor their medical issues. Geriatric Psychiatrists are available in both the in unit and day hospital

• Consultant role for appropriate STAT Day Hospital and STAT Unit inpatients (approximately 3-5 new consultations per week, shared between those two populations)

o Work with the GP MRPs, the interdisciplinary team, and other specialists to optimally assess and treat patients

• Follow the complex medical issues and medication regimes of appropriate patients

• Involvement in interdisciplinary team rounds, care conferences, intake reviews, and family meetings on appropriate patients (approximately 3 hours per week)

• Involvement in the regular intake meetings and in the triage process to identify and prioritize appropriate STAT patients for either the Day Hospital or the Inpatient unit (approximately 2 hour per week)

• Liaison with STAT Centre GP MRP’s, community primary care providers, involved specialists, and pharmacists to facilitate safe, optimal discharges

• Liaison between Inpatient Unit and Day Hospital for appropriate patients who transition from one to the other
