Specialty Nurse Practitioner

Role and Facility Information

The Nexus Primary Care Clinic, located in Nanaimo BC, is looking for Nurse Practitioners to join our team as we build healthier communities and develop a network of outstanding healthcare professionals. Set to open in mid-May 2020 the clinic is operated by the Nexus Patient and Community Care Society in partnership with the Ministry of Health, Island Health and Nurses and Nurse Practitioners of BC, and will adopt the attachment protocol for PCNs to ensure the attachment gap is addressed. In this service contract position the Nurse Practitioner will work as part of a cohesive team with their own unique role and scope in the provision of patient care, working collaboratively to make sure that the patients ‘needs are met in the most effective way possible.

The Nurse Practitioner Primary Care Clinic models the attributes of:

◾ Inter-professional team

◾ Nursing philosophy of care informs all healthcare service provision

◾ Patients will be attached to a most responsible provider within the clinic

and will have access to other care providers most appropriate to meet their

health needs

◾ Same day access

Some outreach is expected

Responsibilities include:

• Comprehensive assessment of patients including diagnosing

diseases, disorders

and conditions

• Initiate treatment including health care management, therapeutic

interventions and

prescribe medications

• Provide medical coordination and participate in multidisciplinary

team planning for the

ongoing health needs of patients

• Participate in the development and operation of a team based practice wide


that may include:

- Health promotion/Disease prevention

- Illness prevention

- Chronic disease management

- Specialty care of specific populations (e.g. frail elders,

terminally ill, pregnant

patients, children)

• Clinical teaching/preceptorship

• Clinical administrative services, including:

- Health care/service planning activities including participating in planning of

longterm health care delivery goals for practice

- Participation in the evaluation of the efficiency, quality and delivery of the

service (e.g. participation in medical audits, peer and interdisciplinary reviews,

chart reviews, and incident report reviews, quality improvement activities).
