Specialty Nurse Practitioner

Role and Facility Information

Our clinic, located in Cranbrook, BC, is seeking a Nurse Practitioner to join our cohesive clinic team. The Nurse Practitioner, as a most responsible provider, offers primary health care services to a panel of attached patients, which includes hospital care within their full scope of practice, (successful candidate will be expected to obtain hospital privileges at the East Kootenay Regional Hospital. EKRH has excellent consultant coverage in all core specialties), including the establishment of a minimum patient panel, and participation in quality improvement activities. The Nurse Practitioner will work collaboratively as a member of the clinic, which will operate within the East Kootenay Primary Care Network, working closely with multidisciplinary healthcare providers to improve the delivery of comprehensive primary care.

Typical Job Duties Include:

Provide full range primary care that includes evening shifts, weekend coverage and participation in a shared group call schedule. You will work in an interdisciplinary team delivering individualized clinical services including assessment, treatment and rehabilitation to clients.

Diagnoses and treats previously undiagnosed patients/clients for undifferentiated diseases, disorders, and conditions within the Nurse Practitioner’s scope of practice; writes orders for treatment and medications; and provides first-line care in emergencies.

Monitors ongoing care, orders appropriate screening diagnostic investigations, interprets reports of investigations, and analyzes information to monitor progress and plan treatment

Establishes priorities for management of health, diseases, disorders, and conditions; provides follow-up treatment; communicates with patients/clients and families about health findings, diagnoses, priorities, outcomes, and prognoses; supports and counsels patients/clients in their responses to diseases, disorders, and conditions.

Be responsible for facilitating discharges from the Emergency Department or acute care of the patients that are within scope for the team, including outreach and participation in an on-call rotation facilitating continuity of care between hospital and community

Prescribes drugs within the statutory and regulatory standards, limits, and conditions for Nurse Practitioners.

Assigns work to other nursing and healthcare personnel, evaluates work, provides education and supervision as necessary and evaluates staff performance.

Maintains population health focus by implementing screening and health promotion activities for populations at risk.

Participates in interdisciplinary staff and nursing education through case presentations; mentoring; role modeling; and facilitating the exchange of knowledge in the clinical setting and the community.

Develops, implements, and evaluates policies and procedures related to nursing, interdisciplinary care, and health system practices.

Performs other related duties as required
