Specialty Nurse Practitioner

Role and Facility Information

The Ministry of Health has recently created Nurse Practitioner (NP) funded positions for NPs interested in starting patient panels within Patient Medical Homes (PMH). This Vancouver clinic is looking to include a Nurse Practitioner to join their clinical team. As part of a multi-disciplinary approach to full service family medicine, an interested NP would work collaboratively in the team environment, and start their own patient panel as per Ministry requirements.

This clinic is a lovely combined family practice and walk-in clinic in the nice upscale neighborhood of Westside Vancouver. They currently have 5 exam rooms, 3 of which can be used as physician/NP offices/exam room fusions. The clinic itself is both a family practice and walk-in hybrid, and is open from Monday to Saturday 8am-8pm, and Sundays from 10:30am-4pm. Shifts are flexible and usually are divided from 8-2/4pm and 2-8pm, and they allow booked appointments for their walk-in patients. Currently, they have space to accommodate a NP up to 7 days a week. There are 5 family doctors working out of this clinic, with 1 to 2 working at any given time. They have a total of 4 MOAs, with 1 to 2 working at any given time depending on the number of family physicians working that day. This clinic is situated in a nice community neighborhood with a mix of working with elderly folk along with some young families. There is free parking and collegial team with excellent support.

• The NP can expect to work full-time with a minimum of 1,680 hours annually over 220 days.

• NP will have the requirement to build a patient panel of at least 500 in year one, 800 in year two and 1,000 is year three

• The contract term is three years

Be part of a cohesive team where each provider has their own unique role and scope in the provision of patient care and you work together to make sure that the patient’s needs are met in the most effective way possible. Responsibilities would include:

• The responsibility and accountability for the comprehensive assessment of patients/clients/residents including diagnosing diseases, disorders, and conditions is key to this role.

• The ability to Initiate treatment including health care management, therapeutic interventions and prescribe medications in accordance with the statutory and regulatory standards, limits, and conditions, and employer policies and procedures.

• The opportunity to practice autonomously and interdependently within the context of an interdisciplinary health care team while providing professional mentorship to other health professionals

• The position collaborates with patients/clients/residents and other health providers to identify and assess trends and patterns that have implications for patients/clients/residents, families and communities; develops and implements population and evidence based strategies to improve health and participates in policy-making activities that influence health services and practices.

• The position participates in peer review and self-review to evaluate the outcome of services at the patient/client/resident, community and population level.

• Current provisional/practicing registration as a Nurse Practitioner with the BC College of Nursing Professionals (BCCNP)

• Relevant, clinical nursing experience
