Our Vision

HealthConnection clinic was established in July 2013 to provide medical, mental health and addictions services to marginalized people who face socio-economic challenges to accessing care. The clinic offers drop-ins and appointments, and a small number of homebound, frail individuals receive home visits. HealthConnection is committed to ensuring North Shore residents have access to the best healthcare possible with an interdisciplinary team of providers and social and addictions supports.

Primary Care medical clinic:

We provide full scope primary care medical home. Clients are attached to the clinic. Our staff include: 2 part time family practitioners and locums), 4 part time Nurse Practitioners , 2 RNs – one an addictions nurse and one a primary care nurse, 2 part time social workers, 1 social support liaison coordinator, 2 MOAs and one part time program lead. A program manager and medical director oversee operations. We also collaborate closely with Mental Health and Substance Use Services, Foundry North Shore, Acute Care providers, Home and Community Care in addition to community partners such as the Lookout Shelter, Canadian Mental Health Association and the RCMP. Clients can self-refer or be referred by other health professionals and community agencies.

The clinic and home visiting operate from Monday to Friday, 8:30-4:30 and includes statutory holidays. There is a plan to expand hours and add a Saturday.
