Specialty Psychiatry

Subspecialty General

Role and Facility Information

Vancouver Coastal Health (VCH) and Vancouver Community Mental Health Services (VCMHS) seek a part-time psychiatrist to work in the new, Integrated Downtown East Side (DTES) model of care. The position requires a minimum of 0.5 commitment (5 half days per week) including some weekends. In addition, this position will be a joint position with Providence Health Care (PHC) and St. Paul’s Hospital who is seeking a .5 commitment (5 half days per week) in the areas of need including the Inner City Youth program.

In alignment with BC Ministry of Health and VCH priorities, Vancouver Community has restructured the delivery of services in the DTES of Vancouver to provide personalized, coordinated care to clients with a consistent team of health care providers. This is an exciting opportunity for a psychiatrist to join a multidisciplinary team of health care professionals in caring for a complex, marginalized population with significant mental health issues. In addition to providing consultations to patients who receive primary care from the VCH, referrals may also come for consultations from private family practitioners working with DTES patients.

The clients often have had multiple psychiatric hospitalizations; unstable housing and/or relationships; difficulties with activities of daily living; high risk for

co-occurring medical conditions; substance use disorder; refractory treatment history; and risk of causing harm to self or others. Outreach to these patients is a part of the model of care and is an expectation of psychiatrists working in the DTES.

St. Paul’s Hospital provides acute and outpatient care to vulnerable populations in the DTES and West End. This position would involve working with younger adults as well as vulnerable populations served by community mental health. There is an expectation of on call at SPH and an interest working with co morbid and refractory illness that would include substance abuse. This component of the position would be reporting to the Department Head of Mental Health at PHC. All PHC physicians are appointed as medical staff to PHC in addition to the VCH appointment and required similar knowledge base and experience.

Reporting to the Medical Director, VCMHS and the local Medical Manager, the successful candidate will provide expertise and consults, act as a resource to primary care providers and case managers, and should have a strong knowledge base in psychopharmacology and behavioral interventions. Additionally, he/she will facilitate continuity of care between hospital and community and liaise with hospital mental physicians and staff. All VCH physicians must be appointed as medical staff of Vancouver Coastal Health; on-call participation is optional.

A clinical faculty appointment with the University of British Columbia Faculty of Medicine is highly encouraged as resident supervision and involvement in education initiatives is an expectation. In addition, psychiatrists working in a DTES Integrated Health Centre will approach care by:

• Placing the patient at the centre

• Utilizing a team based approach, providing consultations as required and longer-term longitudinal psychiatric care when necessary

• Providing culturally safe and responsive care

• Practicing trauma- informed care

• Having a recovery-oriented approach

• Participating in evolving methods in health improvement and care delivery, such as shared care

• Employing the principles of community-based psychiatric care

• Collaborating in managing addictions and concurrent substance use disorders
